I think I can explain that one.  Some federal agents like to chamber a round in their gun, it saves a bit of time in the event you need to draw and fire.  I don't think it would show in the slide if it was chambered.  The guy probably forgot he had the round chambered and...bam, left thigh, lotta pain, more embarassment.
